Issuance of a Foreign Passport to Replace a Lost One – List of Required Documents
The Migration Service of Dnipropetrovsk region explained the procedure for issuing a Ukrainian citizen's passport for traveling abroad to replace a lost or stolen document and the list of required documents.
To obtain a new foreign passport, the applicant must submit the following documents:
- an application form (prepared and printed by the employee receiving the documents);
- Ukrainian citizen's passport (for persons aged 14 and over);
- a document proving the identity of the legal representative, as well as a document confirming their authority;
- birth certificate or another birth document issued by competent foreign authorities. In case the parents or one of the parents were foreigners or stateless persons at the time of birth, an additional certificate of registration as a Ukrainian citizen is submitted (if the passport is being issued for the first time through the Registry);
- documents confirming payment of the administrative fee or a document exempting from its payment;
- one photograph sized 10×15 cm — in case of issuing a passport to a child under 12 years old or a person who cannot move independently due to a prolonged health disorder and requires urgent treatment abroad (with an appropriate medical certificate);
- a statement about the loss or theft of the foreign passport in free form;
- an extract from the Unified Register of Pre-Trial Investigations — in case of theft of the document on the territory of Ukraine.
The agency clarified that if the passport was lost or stolen while the person was in Ukraine and permanently resides abroad, an additional document proving identity and confirming Ukrainian citizenship (or its copy if the original is unavailable) must be submitted, or information about previously issued documents confirming identity and citizenship must be provided.
