What documents are required to insert a photo into the passport-booklet upon reaching the age
The Migration Service of Dnipropetrovsk Region explained which documents need to be submitted to insert a photo into the internal passport of a citizen of Ukraine upon reaching the required age.
The following documents are required to insert a photo into the internal passport of a citizen of Ukraine:
- an application for photo insertion (submitted in person);
- a citizen's passport of Ukraine, 1994 model;
- two photographs sized 3.5 x 4.5 cm with an image corresponding to the person's age;
- if submitting a 1994 model passport issued by a territorial subdivision of the State Migration Service that has ceased operations or temporarily does not exercise its powers, the applicant additionally submits all available documents, including those containing the person's photo;
- a court decision (for persons who did not apply in the manner prescribed by law to insert a photo into the 1994 model passport and do not wish to exchange the 1994 model passport for a passport in the form of a card).
The application review period is five days from the date of submission, but if additional verification of the information provided by the applicant is required, the photo insertion period is extended by no more than 30 calendar days, about which the applicant is informed in writing.
Photo insertion into the citizen's passport of Ukraine is free of charge. The validity period of the passport with the inserted photo is not limited.
