Lost Certificate Before Admission-2026: What Applicants Should Do
Admission to professional and vocational colleges will begin next week, and to higher education institutions — from July 1. Due to the start of the admission campaign, applicants were reminded of the procedure to follow in case of loss of the education document.
If you have lost your document but plan to enroll in 2026 and need a duplicate, please follow this procedure:
1. You need to contact the institution that issued the general secondary education document with an application for the issuance of a duplicate. You can apply in person or through your legal representative — parents or guardian.
2. If the document was issued by an institution currently located in temporarily occupied territories or combat zones, you can apply for a duplicate at any general secondary education institution of the appropriate level at your actual place of residence.
If there is no certificate of the results of the annual assessment of educational achievements, the educational institution conducts an assessment of the graduate's knowledge in accordance with the Procedure for accounting and issuing documents on general secondary education of the state standard to persons who obtained general secondary education in educational institutions in the temporarily occupied territory of Ukraine in 2014, approved by the Ministry of Education and Science order dated May 12, 2014, No. 570.
Please note that for admission, the presence of the document details in the education document registry is required, not the duplicate. Therefore, if the information about the document details is known to you (for example, displayed in the Diia app), it is recommended to request document restoration only after enrollment. Otherwise, the record of the lost document in the registry may be canceled, and the new one not yet created. In this case, admission will not take place.
However, if the document on complete general secondary education was issued before 2000 and lost, admission will be impossible without obtaining a duplicate.
3. If the series and number of the general secondary education document are known, information about it can be obtained through an electronic request to the Education Document Registry of the Unified State Electronic Database on Education (USEDE). The extract is provided with a qualified electronic signature and the seal of the USEDE technical administrator.
To do this, you need to specify the education document data and the personal data of the document owner, the Ministry of Education reminded.
After submitting the request, you can receive:

information displayed on the screen — name, series and number, date of issue of the document, graduate's full name, name and year of graduation of the educational institution, etc. (displayed immediately);

an extract with an applied QES (sent to the email within three working days).
SE "Inforesurs" is the technical administrator of the Unified State Electronic Database on Education (USEDE). In this database, you can obtain information about educational documents, but NOT duplicates of documents.
If the series and number of the general secondary education document are unknown, they can be obtained from SE "Inforesurs." To do this, you need to submit an application and a copy of the passport, and for persons under 14 years old — a copy of the birth certificate. The copy of the document must be certified personally with the inscription "According to the original," indicating full name, signature, and date of certification.
The application can be submitted in paper or electronic form using a qualified or enhanced electronic signature. The application is submitted by the document owner or their legal representative.
Also, if you have a QES (including Diia.Signature), you can use the service "Information about an individual contained in USEDE" and obtain the necessary data. Such information will be displayed if your personal card with the entered RNOKPP (identification code) has been created in USEDE.
