People who were forced to change their place of residence due to the war often lose access to their previous family doctor. The Kyiv City Health Department explains: regardless of the status of an internally displaced person (IDP), patients have full rights to receive medical care and independently choose a primary care doctor.

Medical care in Ukraine is not tied to the place of registration, so IDPs can conclude a declaration with a family doctor, therapist, or pediatrician at any facility that has a contract with the National Health Service of Ukraine (NHSU).

  1. How to choose a doctor

The first step is to decide on a primary healthcare facility. This can be a communal, private institution, or a doctor-individual entrepreneur working under a contract with the NHSU.

You can find a facility through the electronic map of primary healthcare locations using filters:

  • settlement;
  • form of ownership (communal, private, individual entrepreneur);
  • doctor's specialization (family doctor, therapist, pediatrician);
  • doctor's full name (if the patient chooses a specific specialist).

After applying filters, the system shows the address, working hours, contacts of the facility, as well as information about doctors and the number of already concluded declarations.

Established declaration limits:

  • family doctor – 1800;
  • therapist – 2000;
  • pediatrician – 900.

If the limit is not exceeded, the doctor has no right to refuse concluding the declaration. In case of exceeding, the doctor may decide individually.

You can also get information through the NHSU contact center at 16-77 – the operator will help find the nearest facility and doctors accepting declarations.

Concluding a declaration at any facility is free of charge. Demanding payment for this service or the first visit is illegal.

  1. What is needed to conclude a declaration

To register a declaration, you need to make an appointment with the chosen doctor through the reception.

You need to have with you:

  • passport;
  • identification code;
  • child’s birth certificate (if the declaration is concluded with a pediatrician);
  • phone to receive an SMS confirmation code.

During registration, the facility employee enters the data into the electronic health system. The patient receives an SMS with a confirmation code, after which they sign the declaration in two copies.

One copy remains with the patient, the other – at the medical facility.

If a declaration has already been concluded with another doctor, its repeated cancellation is not required – the new declaration automatically replaces the previous one in the system.

  1. What services are available under the declaration

Patients with IDP status who have concluded a declaration with a primary care doctor receive free of charge:

  • consultations and basic diagnostics;
  • prevention of common diseases;
  • e-referrals to specialists;
  • e-prescriptions for medicines;
  • vaccinations according to the immunization schedule;
  • issuance of certificates and sick leaves.

The medical guarantees program ensures equal access to these services for all citizens of Ukraine regardless of IDP status.

Additionally, a declaration in Kyiv communal facilities provides access to the city program "Public Health" for 2026–2028, including expanded vaccination opportunities.

Among them:

  • free flu vaccination for risk groups and medical workers;
  • vaccination against human papillomavirus (HPV) for children aged 12–14, both girls and boys.

An important condition for participation in expanded city programs is having a declaration concluded in a communal medical facility at least three months before applying.
