For many Ukrainians, changing a first name or patronymic is associated with a complicated bureaucratic procedure. In fact, the law allows this to be done at one's own discretion if the established procedure is followed and the necessary documents are submitted. It is possible to change either the first name or patronymic separately, or both personal data elements simultaneously. We explain from what age this is allowed, where to apply, what documents need to be prepared, and how much the procedure costs.

Who Has the Right to Change Their First Name and Patronymic

Citizens of Ukraine have the right to change their first name, patronymic, or both personal data elements at their own discretion. To do this, they must apply to the Civil Status Acts Registration Office at their place of residence and submit the appropriate application.

Individuals who have reached the age of 16 can independently change their first name and/or patronymic. Citizens aged 14 to 15 have the right to submit an application only with the written consent of their parents or guardians.

Where to Apply to Change Your Name

To arrange a change of first name or patronymic, the applicant must personally contact the Civil Status Acts Registration Office with an application and a Ukrainian citizen's passport.

Persons aged 14–15 must additionally submit written consent from their parents or guardians.

What Documents Are Needed to Change First Name and Patronymic

In addition to the application and passport, it is necessary to submit:

  • birth certificate;
  • marriage certificate (if the applicant is married);
  • divorce certificate (if the marriage has been dissolved);
  • children's birth certificates (if children are present);
  • certificates of name changes of the applicant, father, or mother if such changes were made previously;
  • photograph;
  • a written commitment acknowledging awareness of the legal responsibility for providing false information and the necessity to exchange the passport and other documents in case of a name change. The document is drawn up in free form, and the authenticity of the signature must be notarized;
  • receipt of payment of the state fee.

How Much Does It Cost to Change a Name in Ukraine

For the state registration of a change of surname, first name, and patronymic (except cases related to marriage registration), a state fee of 5.10 UAH must be paid.

For a repeated change of surname, first name, and patronymic not related to marriage registration, the state fee is 51 UAH.

How Long Does It Take to Process the Application

The consideration of an application to change the first name and/or patronymic takes up to three months from the date of submission.
